Local forms in Word format:

Note: If you have Microsoft Word, you should be able to call up these forms and fill in your information. When you are done, you can either save the file to your own disk space or print it out for submission. You cannot save your information to the web server, so don't be concerned about your personal information getting out on the web. If you are concerned about saving particularly personal information on a computer disk, you can leave that blank and type it in on a typewriter or print it in carefully later.

• Selective Service Registration Form:  Male students are required to register within 30 days of their 18th birthday and can do that either in the process of submitting a FAFSA form or by going to the web site below.  You can also find paper forms at the local post office.
